Extend will also not cause the problem. I can slide spinning graphics from one monitor to the other, no stutter. It seems it can only happen if my laptop screen is off in some way
 
I cannot alter the refresh rate of the second monitor there is only one option. Changing the refresh rate of my laptop monitor does not affect anything.

If it helps, the stuttering is very bad and noticeable. It is not micro-stutter
 
People have been having an almost identical problem for many many years. The two forum posts I found were on Intel and Nvidia websites.


One of the (suspect) solutions was to upgrade to Windows 11. I doubt this will work, but I upgraded to Windows 11 to see if things will be resolved.
 
Windows 11 seems to 90% fix the problem. It appears to be a known issue, but since it has to do with a combination of Intel, Nvidia, the Laptop Manufacturer, and Microsoft, everyone has been saying "not my problem" for years. It happens when a laptop of a specific brand (Lenovo, MSI, others) has both integrated graphics and a NVIDIA chip, and gets connected to an external monitor.

But as I mentioned Windows 11 seems to resolve 90% of it, and the remaining 10% micro stutter I can deal with I suppose.
 
yeah, I had something similar with an Intel 600P series m.2 drive.
that component in combo with Win10 and a gigabyte mobo stopped me getting Windows Updates or something equally non-critical that I could live with.
after waiting a couple of years, ended up needing a larger drive so problem was 'solved' by making sure I didn't get Intel again.
sounds like you may be in a similar boat!?
 
having reached an impasse due to hardware, I'll close this now.
