here 700W seems a bit ott to me and I feel that a good quality brand 650W Gold efficiency rated PSU would be adequate, fingers crossed for you that a weak PSU has not damaged your card, is there anyone that you can ask to test the GPU in their system for you, alternative being to ask a local tech/tech store what they would charge to test the card for you.
You have a couple of other equally important things going on according to Speccy but if you wish to address them while waiting to upgrade the PSU I would strongly suggest that you disconnect your second screen so as to lighten the load and drop the internal temps of the PSU.
