Make sure to select Normal startup from System Configuration.
I have notice you have three sound cards try select different playback device from Control Panel → Sound.

The issue is now resolved; it turned out not to be a software issue at all. In the end, I found that my CPU had been damaged (two corners were severely bent) by the CPU cooler I chose for my build (a Zalman CNPS9900MAX); even though the cooler’s documentation claimed it was compatible with socket 1151, the mechanism attaching the cooler to the motherboard required too much force, which ended up destroying the CPU and motherboard. I ended up replacing both this weekend (and used a Cooler Master Hyper 212 EVO cooler), and my PC is back to normal now.
