Hi there my system is old, but it worked without problems till this year I've tried every possible fix except changing hardware, not possible at this moment. When playing games, sometimes it happens after a couple of weeks, and sometimes it happens countless times in one month.

Have you tried DISM then SFC.

Created a different user account on the system to see if the issue replicates?

Run the system in Clean Boot?
 
Yes I did run DISM.exe /Online /Cleanup-image /Restorehealth and I did clean boot still not helping.
 
I begin to think my 6 pin power cable has something to do with it because sometimes when I boot up my pc it will not post and when I wiggle the power cable and restart then it posts.
 
I'm having the exact same problem as well.
- Reimaged to clean state
- RMA'd GPU
- Replaced display port cable
- Replaced PSU

I have new ram coming, but I really don't feel like that is what is causing it, ran memtest86 with zero errors after 4 passes all tests as well.

So replacing my GPU doesn't mean it will be fixed because you RMA'd your GPU and you still have the error? Man, that makes me worried.
 
@XHIHOX if you are asking for assistance, it is disrespectful to ask on someone else's thread you will have to create your own to receive assistance since everyone issue results will be different.
 
It could be the six pin that is the issue if there is a wire that is not making proper connection. would replace before using the system again to prevent possible damage to hardware if not already starting.

Have you tried reinstalling the GPU drivers using DDU to remove then current one completely. Will need to download new ones.

Does the same thing happen if the Display Card is removed, and the onboard graphics are used?

Yes did run DDU and clean install new Nvidia drivers, also formatted Windows I take all my Ram and cleaned then reseated it.

You installed the Chipset and then the storage/SATA drivers and third the graphics drivers, the drivers can either come from a disk provided by the motherboard manufacturer or downloaded from their site and saved to a flash drive etc, this is a must and Windows should not be allowed to check for updates before it has been done as more often than not Windows installs the wrong drivers or in the incorrect order and this can cause all sorts of problems.

Have you tried one of the other display sockets on the MB.

Still missing answers to some question asked in previous post.

Well I have a folder with all my drivers that came with the pc the Chipset, storage etc... And i always install them first then Nvidia, and then I connect to the internet, so windows can update after. And for the other display sockets when I do reseat my GPU in another socket I can get maybe like 2 weeks without errors then the error will come back randomly. And yes I did run the DISM and SFC did a clean boot also made another account and tried that also.
 
I changed the 6 pin cable and also changed my NVIDIA from maximum performance to optimal performance, haven't crashed yet but will keep you updated.
